GUIDELINES IN SECURING CLEARANCE:

  1. Students whose names appear on the LIST OF STUDENTS WITH ACCOUNTABILITIES should:
  • get a clearance slip from Chitty Nepomuceno of the HS Principal’s Office (HSPO). The clearance slip will indicate the office/s where the student still has accountability.
  • settle the accountability, pay dues at the Accounting Office (if necessary), and secure the signature of the authorized personnel from the office/s concerned
  • submit the accomplished clearance slip back to Ms. Nepomuceno (HSPO)
  1. STUDENTS WHO ARE NOT CLEARED WILL NOT BE ABLE TO CLAIM THEIR REPORT CARD ON MARCH 25, 2015.

GUIDELINES IN FILING REQUEST FOR REVIEW OF MARKS:

(Requests should be filed within 3 days after posting of grades)

  1. Student discusses the petition with the subject teacher concerned
  2. If the petition cannot be settled at this level, (student-subject teacher), the student elevates it to the Department Chairperson concerned.
  3. However, if the appeal is not settled and the student wishes to further pursue his appeal, he must secure the ACADEMIC APPEAL FORM, which can only be obtained from the Department Chairperson Concerned.
  4. The student is required to substantiate his appeal and then, submit it to the Office of the Assistant Principal for Academics.  (Note:  There should be a clear basis of the justification for appeal.  If necessary, pertinent documents may be attached)
